New Patient? How to transfer your records

As a New Patient, it is helpful to the Doctor and Hygienist to have dental records from your previous dentist(s) to review the treatments that were prescribed in the past and to be made aware of trends and concerns that may impact your current and future treatment.

Prior to your first visit, we recommend that you contact your former dentists(s) and request your records and x-rays be released to our office.  In most cases your former dental home will want this request in writing.  For your convenience a Patient Record Release form can be printed from the Dental Forms section of this website and mailed, faxed or emailed directly to your former dentist.  Of course we suggest that you contact your former dentist to ask if they have a preferred means of requesting records transfer. 

Since most dental offices have digital radiographs, it is easy for your former dental provider to email the pictures to our email address: This email address is being protected from spambots. You need JavaScript enabled to view it..  At the push of several buttons, Dr. Jungblut will have your information available to her to compare with her findings at your initial examination.
